Become an HFM Patient

It's easy to become a Heritage Family patient. We do require some information about you so please complete the forms below, preferably PRIOR to your initial office visit. Additionally, in order to get the most out of your office visits, we encourage you to read the article, How to Be a GREAT HFM Patient.

Five Easy Steps to Becoming an HFM Patient:

1. For New Patients:

Schedule your first HFM appointment by phoning our staff at 360-704-2362 (704-ADOC). If you have scheduled your own appointment online we still request that you please call our office before arrival to be certain you understand our financial policies and which insurance companies we are currently fully participating with. Next, please fill out these four forms so that we have a complete picture of your overall health, family, and personal conditions:

2. HFM Online Patient Pre-Registration Form

Complete this shortened "getting to know you" secure form, online. You will then finish this form at our office – please allow about 15 minutes before your appointment. However, you may download the full HFM Registration Form to complete and bring with you on your first visit. This form is completed before your first visit and reviewed for accuracy at your first visit in each new calendar year. -- Adobe Acrobat Reader is required, (a download link is provided below).

3. Pre-Appointment Questionnaire (PDF file)

Download, print out, complete this form and bring it with you to your appointment. You may also complete this form at our office. BE SURE to list or bring with you a list of all your current medications and supplements. -- Adobe Acrobat Reader required,(a download link is provided below).

4. Symptom Review Form (PDF file)

Download, print out, complete this form and bring it with you to your appointment. You may also complete this form at our office. This form will ask you to mark off any symptoms you have experienced in the past six weeks. This form is completed before your first visit and about yearly thereafter. -- Adobe Acrobat Reader required,(a download link is provided below).

5. Family, Personal and Social History Form (PDF file)

Download, print out, complete this form and bring it with you to your appointment. You may also complete this form at our office. This form will provide us with additional information that will allow us to have a full picture of any and all issues that may impact your health. -- Adobe Acrobat Reader required,(a download link is provided below).

If billing insurance, BE SURE to bring your insurance card with you to your appointment.
