

To ensure SECURE communication at every step of the process, e-Consults and e-Rx Refills are initiated through the online appointment system. You MUST make a "pretend" appointment under the e-Consult or
e-Rx Refill services.
Intructions for e-Consults and e-Rx Refills
Because it can take the physician and staff as much (or more) effort and time to address an e-Consult or
e-Rx Refill as a face-to-face encounter a charge is made for most as follows.
e-Rx Refill request $10.00
Basic e-Consult w/physician $25.00
Extended e-Consult w/physician $40.00
In no circumstances will charges for e-Consults or e-Rx Refill requests exceed $50.00.
A credit or debit card number must be entered to submit a e-Consult or e-Rx Refill request. NO charge will be applied to your card for simple questions regarding the subject of a recent visit or for a prescription refill that should not have run out yet.
A birth date (mm/dd/yyyy) and e-mail address is required for e-Consults and e-Rx Refills.
How to use the e-Consult system to ask a medical question:
1. Login to the system
2. Choose a "Practitioner" from the drop-down list
3. Choose "e-Consult" under the services drop-down list
4. Choose the first available day on the calendar
5. Choose the next available time slot on the appointment calendar
6. Enter your birth date, e-mail address, and then your medical question in the "Special Instruction" box
7. Continue with billing information as instructed on the screen
Your practitioner will send a secure response (to the e-mail address given with your e-Consult) via MessageguardTM secure messaging systems. You will be able to respond securely as with a regular
e-mail "conversation." Please allow two business days for a response(s) - though we expect to do better than that. You will be notified of the charge, if any, to your credit card, when the consutlation is completed.
How to use the e-Rx Refill request system:
A $10.00 charge will apply to "overdue" medication refills that typically would have been handled in an office visit. You will not be charged for medication refills necessary to complete a previously agreed upon course of treatment.
We need to know the following to process your e-Rx Refill request
Birth date (mm/dd/yyyy)
Email address
Name of medication
Strength of medication (usually in mg.)
Quantity (how many do you seek?)
Instructions for taking (i.e. three x per day)
Next anticipated appointment
The pharmacy you would like the e-Rx Refill called in to
Did HFM make a mistake in quantities? There is no charge where insufficient quantity (quantity and refills, together) were provided by HFM. If you believe the refill charge should not be applied please tell us why that is so (i.e. "you called me and told me to double up, so I am running out" or "Supposed to return in 6
months, but my prescription will not last that long.")

4. Choose the first available day on the calendar
5. Choose the next available time slot on the appointment calendar
6. Enter your birth date (mm/dd/yyyy), e-mail address, and then your refill request information as listed
above, in the "Special Instructions" box
7. Continue with billing information as directed on the screen
You will be notified by e-mail the simple message that "your prescription has been authorized" (or not, or perhaps partially refilled) and any charges applied to your card. As this content is not by most people considered particularly "sensitive" we will use REGULAR e-mail for this notice as it is easier to manage for both HFM and for our patients unless you tell us in your initial request to use only the secure MessageguardTM system. Please allow two business days for a response - though we expect to do better than that. You will be notified of the charge, if any, to be applied to your credit card when the consultation is completed.
